A "name mismatch" almost always means the license was generated for a different string than the one the program is currently using. The check is character-for-character, including case, spaces and any trailing space — so "MaxMan301" and "maxman301" are two different licenses.

What to check first

Open the activation window and compare the name field there with the name you posted in your license request, character by character (watch for a leading/trailing space from copy-paste).
If you regenerated activation.req after the first request, the name baked into the new request may differ from the one the key was issued for — in that case the old key won't validate.
Make sure only one license file is present in the program folder; a leftover file from an earlier request can be picked up first.

Note on issuance
